"The Facility Mysteries"

An autistic boy goes missing from his school for weeks. A patient admitted to the hospital goes missing  only to be found dead for 17 days, in the hospital stairwell. There is an expectation of safety by parents leaving their children in the hands of school officials, and a reasonable expectation of safety when you leave your loved one in the hands of a medical staff.

What causes these incidences of gross negligence, and who's to blame?
